@aral You should replace "capitalism" with neoliberalism. I believe that there are social forms of capitalism that can work very well.
I don't know. I can't think of any version of capitalism that won't result in the very wealthy. I agree there may be versions of capitalism that can work well, but I don't think that's the same thing as not creating billionaires.
I think capitalism requires a tolerance for a relatively high level of inequality, else it's shifted passed the (notoriously difficult to define) system of capitalism into something else.
